半胱氨酸蛋白抑制剂C:同型半胱氨酸、超敏C—反响蛋白和D—二聚体联合检测在脑卒中确诊和预后判别中的使用价值

[摘要] 意图 讨论同型半胱氨酸(Hcy)、超敏C-反响蛋白(hs-CRP)和D-二聚体(DD)联合检测在脑卒中确诊和预后判别中的使用价值。 办法 选取本院2013年5月~2014年9月收治的224例脑卒中(脑出血96例,脑梗死128例)患者作为研讨目标,选取同期的115例健康体检者作为健康对照组,比较各组的Hcy、hs-CRP和DD水平。 成果 脑卒中组的Hcy、hs-CRP和DD水平明显高于健康对照组,差异有统计学含义(P<0.05)。脑出血组与脑梗死组的Hcy、hs-CRP和DD水平明显高于健康对照组,差异有统计学含义(P<0.05)。脑卒中组的Hcy、hs-CRP与DD联合检测的阳性率为91.5%,明显高于健康对照组的3.5%,差异有统计学含义(P<0.05)。脑卒中组Hcy、hs-CRP、DD检测的阳性率明显高于健康对照组,差异有统计学含义(P<0.05)。 定论 Hcy水平是脑卒中的重要猜测因子,Hcy、hs-CRP和DD联合检测有助于脑卒中的前期诊治,而且对判别预后也有重要的临床价值。

[Abstract] Objective To explore the application value of combined detection of homocysteine (Hcy),hypersensitive C- reactive protein (hs-CRP) and D-dimer (DD) in the diagnosis and prognosis of stroke. Methods 224 cases of stroke (96 cases of cerebral hemorrhage,128 cases of cerebral infarction) from May 2013 to September 2014 in our hospital were selected as the research object,115 healthy persons in the same period were selected as the healthy control group.The level of Hcy,hs-CRP and DD in the each group was compared. Results The level of Hcy,hs-CRP and DD in the stroke group was higher than that in the healthy control goup,with significant difference (P<0.05).The level of Hcy,hs-CRP and DD in the cerebral hemorrhage group and the cerebral infarction group was higher than that in the healthy control goup,with significant difference (P<0.05).The positive rate of combined detection of Hcy,hs-CRP and DD in the stroke group was 91.5%,which was higher than 3.5% in the healthy control goup,with significant difference (P<0.05).The positive rate of Hcy,hs-CRP and DD in the stroke group was higher than that in the healthy control goup,with significant difference (P<0.05). Conclusion Hcy level is an important predictor of stroke,combined detection of Hcy,hs-CRP and DD is helpful for early diagnosis and treatment of stroke,and which also has important clinical value in judging prognosis.

脑卒中病况严重,预后差,是一种严重影响人类健康的常见病。研讨显现[1],血清同型半胱氨酸(homocysteine,Hcy)和超敏C反响蛋白(hypersensitive C-reactive protein,hs-CRP)目标关于脑卒中发病的防备及病况剖析具有重要含义。Hcy和hs-CRP是动脉硬化和缺血性脑卒中的独立风险要素[2],而且血液成分(高凝状况)和血流动力学的改动也是其发病的重要原因之一[3]。D二聚体(D-Dmier,DD)是纤维蛋白降解产品中的最小片段,是反映凝血和纤溶活化的分子标志物。本研讨经过检测Hcy、hs-CRP和DD水平,讨论其在脑卒中确诊及预后判别中的使用价值。

1 材料与办法

1.1 一般材料

选取本院神经内科2013年5月~2014年9月收治的224例脑卒中患者作为研讨目标,均参照1995年全国第四届脑血管病学术会议经过的确诊规范[4]确诊,扫除肝、肾功能不全等患者,其间脑出血96例,脑梗死128例。脑出血组中,男54例,女42例;年纪46~77岁,均匀63.2岁。脑梗死组中,男75例,女53例;年纪43~83岁,均匀68.7岁。两组的性别、年纪等一般材料比较,差异无统计学含义(P>0.05)。选取同期的115例健康体检者作为健康对照组,其间男64例,女51例;年纪47~76岁,均匀62.5岁。本研讨经本院医学道德委员会赞同,均获得患者知情赞同。
